Our health clerk is Lori Montgomery, and our school nurse consultant is Liz Eberhard RN. The health office is open 7:30 am-4:00 pm Monday through Friday. During this time, Lori is in the health office. Liz is in the building once a week (usually on Thursday).
The health office serves multiple functions. We see students and staff for illness or injury during the school day, administer prescribed medications, and maintain medical and immunization records. Additionally, we can serve as an interface between health care providers and teachers. If your student has health issues, let us know, we can help!
If your student needs medication or treatment while at school, we can help. We need a completed medication authorization form from an approved health care provider (physician, nurse practitioner, or physician assistant) with a parent's signature. Bring the authorization and medication (in pharmacy labeled packaging) to the health office to get started. Provider authorization forms can be found below or on the District 6 Health Services page under
Student Health Concern Information and Forms.
Please complete the health information on the online registration form, particularly if your student has health needs.
