    WHAT IS 18-21 TRANSITION SERVICES?


    18-21 Transition Services is a community based program that supports students in increasing their independence and obtaining employment.

     

     

    WHO QUALIFIES 


    Students who are between the ages of 18-21, have met graduation credit requirements, and who continue to have transition needs as they relate to the individual’s post-secondary goals of their IEP.

     

     

    VISION 


    18-21 Transition Services offers:

    • Programs that are based on employment, independent living skills, and agency resources within the community.
    • Approaches that are open-minded, creative, and collaborative with students, parents, agencies, and staff.
    • A culture that is welcoming, responsive, and committed to continuous improvement in building self-reliance.
    • Students are respected and treated as full partners in the process of increasing independence.
