18-21 Transition Services assists individuals with disabilities to become successful in their community by gaining independent living skills, effective communication, and responsibility within their job.
WHAT IS 18-21 TRANSITION SERVICES?
18-21 Transition Services is a community based program that supports students in increasing their independence and obtaining employment.
Students who are between the ages of 18-21, have met graduation credit requirements, and who continue to have transition needs as they relate to the individual’s post-secondary goals of their IEP.
18-21 Transition Services offers:
- Programs that are based on employment, independent living skills, and agency resources within the community.
- Approaches that are open-minded, creative, and collaborative with students, parents, agencies, and staff.
- A culture that is welcoming, responsive, and committed to continuous improvement in building self-reliance.
- Students are respected and treated as full partners in the process of increasing independence.
