Leave of Absence Information
If you are needing to request a leave of absence that is not considered descretionary, vacation, or sick leave, please see the following steps that need to be taken.
1- Submit the LEAVE OF ABSENCE FORM
2 - Human Resources / Risk Management will contact you with any further questions they may have
3 - You will be notified of approval or denial of leave
