• Small Vehicle Permit Obtaining a Small Vehicle Permit (SVP) to transport students in a District Activity Bus, District Sponsored Rental, or Personal Vehicle

New Applicant

  • Fill out the Motor Vehicle Record request (MVR) and the Medical Information form, and send it to Training at Transportation via email sterrill@greeleyschools.org, department mail or fax (970-348-6830). Note that any "Yes" answers on the medical form will require a doctor's release to drive students and return to Transportation. Transportation will notify you whether or not your MVR and Medical form meet the required criteria set forth by the CDE and the district's insurance carrier. This step must be completed prior to operating any district vehicle. 

     

    Required study materials are listed on the SVP Personal/Acknowledgement form. It is your responsibility to read/watch all of the listed materials and initial each line indicating you completed each prior to contacting the training department at Transportation to schedule a time for your written test(s). Once written testing is complete, we will schedule the pre-trip and driving test which cover information specifically related to the CDE Type A Multifunction Bus/Small Vehicle Operator Guide. 

    Please send all signature pages from the required training to Shadra Terrill in the training department prior to testing. 

    On the district website, you will also find a link to the current CDE online SVP course. You will need to email CDE, get login information and complete the online course. Once that is completed, you will need to print and provide a copy of the certificate to training at Transportation.

     

    For your pre-trip and drive tests, it is beneficial to review the Colorado Driver's Manual. 

    A Small Vehicle Permit will be issued upon successful completion of all paperwork, written test(s), pre-trip and road test. You will be required to show it any time you use a district vehicle to transport students. 

     

    The procedure for renewal of the SVP requires the same MVR and Medical forms as above, a current copy of your drivers' license, and the CDE written test. No pre-trip or road test is required if the SVP is renewed before it expires. 

     

    If the SVP expires before the renewal process has begun, all of the previous steps must take place and a pre-trip and road test is required. 

Renewals

  • Plan ahead, this process takes time! The SVP is good for one year from the date of completion. It may only be renewed for two years.

    You are required to complete and initial the SVP Personal/ Acknowledgment form, indicating you have completed each listed required training. The only written test required is the annual CDE written test. If you do not have a current CPR/First Aid card, the First Aid test will be required.

    A Small Vehicle Permit will be reissued upon successful completion of all paperwork and written test(s). You will be required to show it any time you use a district vehicle to transport students. 

     

    If the SVP expires before the renewal process has begun, all steps for a new permit must take place and a pre-trip and road test is required. 

     

    All steps must be completed prior to operating any district vehicle.
