Steps for a Replacement Badge
- LOST or STOLEN BADGE: Immediately report a lost or stolen badge to your Office Manager or Supervisor.
- REPLACEMENT/TRANSFER BADGE: Place a request with your Office Manager or Supervisor.
- Facilities Department personnel will process the request upon receiving the appropriate information.
- Please note: If the photo on file is 5 years or older, a new photo must be taken by the Human Resources Department before a new badge can be created.
- Once a new badge is created and programmed, it will be delivered to your Office Manager or Supervisor.
- Try the new badge as soon as possible to ensure it works properly.
- The PIN will remain the same unless specifically requested for it to be changed.
- Do NOT write the PIN on the badge
