    CDC recommends wearing cloth face coverings in public settings where other social distancing measures are difficult to maintain (e.g., grocery stores and pharmacies), especially in areas of significant community-based transmission.
    How to Wear a Cloth Face Covering
    Cloth face coverings should—
    • fit snugly but comfortably against the side of the face
    • be secured with ties or ear loops
    • include multiple layers of fabric
    • allow for breathing without restriction
    • be able to be laundered and machine dried without damage or change to shape

    Welcome to the Chappelow health office.     Our health clerk is Rose Downs and our school nurse consultant is Liz Eberhard RN.  The health office is open 8:00 am-3:30 pm Monday through Friday.  During this time, Rose is in the health office.  Liz is in the building once a week (usually on Thursday).
    The health office serves multiple functions.  We see students and staff for illness or injury during the school day, administer prescribed medications, maintain medical and immunization records.  Additionally, we can serve as an interface between health care providers and teachers.  If your student has health issues, let us know, we can help!
    If your student needs medication at school, we need a completed physician authorization form, with a parent signature.  Bring the authorization and medication (in pharmacy labeled packaging) to the health office to get started.  Physician authorization forms can be found below or on the District 6 Health Services page under Student Health Concern Information and Forms.
