Special Dietary Needs

Nutrition Services works hard to provide for all students throughout the district whatever their needs might be. Below you will find links to documents that will help you to ensure that your student receives meals that are safe and healthy.

Special Dietary Needs Accommodation Request Form

Please note that the Special Dietary Needs Documentation Form must be completed by a recognized medical authority and returned to the Kitchen Manager (at the site your student attends) in order for meal substitutions/accommodations to be evaluated and provided. Download Accommodation Request Form

Explanation of Requirements for School Meal Modification Requests

Dear Parent/Guardian:

Your child’s school:

  • Will make meal modifications prescribed by a licensed physician, advanced practice nurse with prescriptive authority, or physician’s assistant to accommodate a disability. Properly completed and signed paperwork is required as noted below.
  • Will make meal modifications prescribed by a medical authority due to a food allergy/intolerance or other medical condition that does not rise to the level of a disability. Properly completed and signed paperwork is required as noted below.
  • Will not make substitutions for fluid cow’s milk due to a food allergy/intolerance unless documented as a disability by a prescribed medical authority. Milk is not a required meal component and can be declined by any student.
  • The required Special Dietary Needs Documentation Form can be obtained at the link above or from the Kitchen Manager, School Nurse, or Health Clerk. The form contains two sections: one to be completed for students with disabilities or handicaps; and the other to be completed for students with other special dietary needs.
  • Disabilities/Handicaps: Any person who has a physical or mental impairment which substantially limits one or more major life activities. Please note that severe food allergies (e.g., life-threatening peanut allergies) fall into this category.
  • Other Special Dietary Needs: Food allergies (that are non-life-threatening) and food intolerances fall into this category, as do other medical conditions that do not rise to the level of a disability/handicap.

Only the types of meal modifications explained in the first paragraph of this letter are applicable to your child’s school.

To ensure the requested meal modifications can be made as soon as possible, return the completed Special Dietary Needs Documentation Form to your school’s Kitchen Manager, School Nurse, or Health Clerk.

Peanut Cautious Disclaimer

Weld County School District 6 (WCSD6) Nutrition Services is committed to student safety and wellness. This document outlines our approach to being peanut cautious, rather than peanut free.

Some WCSD6 schools have elected to become designated peanut-cautious sites. At these locations, Nutrition Services substitutes peanut-containing menu items with alternatives that do not contain peanuts or tree nuts.

Important Notes:

  • WCSD6 Nutrition Services sources products from a wide range of vendors and manufacturers.
  • While we ensure that products served at peanut-cautious sites do not contain peanuts or tree nuts as ingredients, many manufacturers cannot certify that their production facilities are completely free of peanuts or tree nuts.
  • Therefore, while the items served are intended to be peanut/tree nut free, cross-contact during manufacturing may still be possible.
