Please complete the survey below to assist us in understanding how your student accesses the internet at home. You will only need to complete it only once, but it will ask you to list all students in your family so we have plans for each and every student. If you have any questions, please feel free to reach out to me. The survey can be completed in English or Spanish and we would ask that you complete the survey as soon as possible.
