FREE Immunization Clinics
Greeley-Evans School District 6, in partnership with the Weld County Department of Public Health and Environment and Sunrise Community Health, is hosting several free TDAP immunization clinics for incoming sixth graders.
The TDAP vaccine for tetanus, diphtheria and pertussis is required for entry into sixth grade by Colorado immunization laws.
Here are the scheduled clinics:
- Tuesday, August 6: 8 a.m. – 6 p.m. at Heath Middle School, 2223 16th Street in Greeley
- Tuesday, August 6: 8 a.m. – 5 p.m. at Franklin Middle School, 818 35th Avenue in Greeley
- Tuesday, August 6: 8 a.m. to noon and 1 p.m. to 6 p.m. at Prairie Heights Middle School, 3737 65th Avenue in Evans
- Tuesday, August 13: 4:30 p.m. to 7:30 p.m. at Brentwood Middle School, 2600 24th Avenue Court in Greeley
A parent or legal guardian must be present to complete required consent forms. Parents and guardians are also asked to bring the most current copy of the student’s immunization record. All District 6 sixth graders are invited to attend, regardless of the school in which they are enrolled.
