Nurse Family Partnership – Pregnancy Resource
The Nurse-Family Partnership program is a no cost, 2 1/2 year program that serves qualified low income, first time mothers. Enrollment occurs during pregnancy and continues until the child is 2 years old.  Specially trained registered nurses visit the clients in their homes to help them develop healthy nutritional habits, positive relationships, and safe living environments. Clients are encouraged to finish school and to set goals toward achieving financial independence.  Parenting skills and normal childhood growth and development are taught through handouts, DVDs and hands on demonstrations.
SCHOOL NURSE can make a referral and the program nurses will contact you.

Healthy Baby Campaign – Pregnancy Resource
The Healthy Baby Campaign is a free educational program for pregnant women of all ages who reside in Weld County and for local organizations who serve pregnant women and their families (including physicians, teachers, social workers, law enforcement agents, faith-based groups, and other businesses).  If you would like to join the Healthy Baby Program because you are a pregnant woman, or if you work for an organization that serves pregnant women, contact information is listed below.

Lutheran Family Services
LFS is a statewide multi-service human care organization with regional offices in Greeley, Fort Collins, Denver and Colorado Springs providing services to 20,000 people annually through adoption, foster care, individual and family therapy, prevention services, older adult services and disaster response.  LFS has served Colorado communities and families since 1948 regardless of race, ethnicity, religion, age, sex, gender identity, sexual orientation, disability, military/veteran status or economic status.  They teach many parenting, teen parenting, and family relationships courses.

Healthy Communities Program

The Healthy Communities Program is for children 0-20 and pregnant women. The program provides application assistance to pregnant women and families that may be eligible for Medicaid and/or CHP+. The program also assists those already receiving Medicaid and/or CHP+ in understanding their benefits and accessing medical, dental, vision, mental health and specialty services. They help pregnant women find providers for their prenatal care, helping to ensure a healthy pregnancy and a healthy baby.

  • Medicaid and CHP+ benefit information
  • Referrals to Medicaid and CHP+ providers
  • Assistance with eligibility issues
  • Checking status 
  • Referrals to community resources
